Guy’s and St Thomas’ Hospital empowers remote monitoring for 1,900+ patients with Unboxed

Guy’s and St Thomas’ Hospital needed a better way to monitor its patients with chronic conditions like rheumatoid arthritis between appointments. The existing paper-based system was inconvenient for stable patients and made it difficult for clinicians to identify and assist those whose symptoms were worsening. The hospital commissioned the vendor Unboxed to explore a user-centred service transformation.

Unboxed developed a remote monitoring service using an agile, iterative approach. The solution features an online form for patients to self-report symptoms from home, managed via GOV.UK Notify, and a dashboard for clinicians to track changes. This service, built through extensive collaboration with patients and clinicians, achieved a 70-79% completion rate for online reports and registered over 1900 rheumatology patients. The success has led to the service being scaled for use with long Covid patients and GPs.
