Using Analytics and Strategic Planning to Execute on Value-Based Care Initiatives

Johns Hopkins Medicine created its Office of Population Health to improve population health, reduce health disparities, and execute value-based care initiatives across the health system. To support this effort, Johns Hopkins Medicine needed a better way to identify high-risk patients, coordinate care across departments, and build a long-term strategy for lowering unnecessary utilization. SS&C Technologies, through the Johns Hopkins ACG System, helped provide the analytics foundation for this work.

SS&C Technologies supported Johns Hopkins Medicine with population health analytics that flagged high-risk and high-utilizing patients, including those with frequent hospitalizations, ED visits, and elevated risk scores. The OPH used these insights to create a biweekly master list, improve coordination between inpatient and outpatient teams, refine workflows, and guide a multi-year strategic plan focused on bending the utilization curve and reducing health disparities. The result was a more data-driven, coordinated approach to care delivery and value-based care performance across Johns Hopkins Medicine.
