Health Leads is a nonprofit that helps doctors “prescribe” basic resources like food, housing, and heat and mobilizes nearly 1,000 college student advocates across 20 health centers to connect patients with those resources. Faced with fragmented spreadsheets and a standalone database that made data entry, sharing, and tracking client progress difficult, the organization needed a secure, centralized system to manage clients, community resources, and advocate activity.
Health Leads migrated to Salesforce—using Sales Cloud for relationships and a customized, HIPAA-compliant Community Cloud for advocate intake, case management, and collaboration—accessible on iPads and via the Salesforce Platform app. The new system centralizes client and resource data, enables secure communication and coordination with providers, and gives staff the tools to measure impact, helping the organization support more families and deliver more effective, coordinated care.
