This enabled real-time alerts to care teams, facilitating immediate intervention and confidential coordination with a substance use response team. The solution improved the identification of previously undiagnosed overdoses and allowed CHOMP to successfully connect with difficult-to-reach patients to coordinate critical follow-up care, all while supporting a larger institutional effort to reduce stigma and ensure fairer treatment.
