North Kansas City Hospital (NKCH), a 451‑bed regional health system with more than 3,000 employees and the Meritas Health network of physician practices, faced the challenge of retaining steerage of health plan dollars and improving the health of over 6,600 covered lives. Leadership sought to develop population health management programs, implement a patient‑centered medical home shared‑savings initiative, and quantify the impact of interventions while stabilizing benefits and utilization.
Using the Advanced Plan for Health (APH) data‑analytics risk engine, NKCH aggregated and stratified population risk, targeted preventive care and medication adherence, and launched a wellness program with an on‑site coordinator (HRA, biometric testing, individual coaching) plus disease management and monthly performance reviews. The program drove measurable results: 85% annual HRA completion, 60% full wellness participation, 80% of adult enrollees using an integrated PCP, and more than $200 million in cumulative health care savings over 12 years while maintaining stable benefits.
