Transitional Care Clinic and post-discharge calls boost patient-centered care effectiveness and cost savings

Via Christi Health, Kansas’ largest provider of health care services, needed a more consistent way to support patients after discharge and reduce readmissions. Its decentralized callback process pulled busy staff away from the bedside and made it harder to identify patients who needed additional help, especially those without access to primary care or facing barriers such as chronic illness, insurance issues, or limited resources.

To address this, Via Christi Health partnered with **NRC Health** and implemented **Transitions** along with a centralized discharge call process, integrating it with the Transitional Care Clinic. The program helped contact nearly 19,000 patients in the first nine months, identified 3,200 needing follow-up, and reduced readmission rates from 9% to 5% among participants. Via Christi also reported a 93% reduction in ED and inpatient utilization for Transitional Care Clinic patients, an estimated $1.6 million in cost avoidance, and a 98% reduction in emergency visits after expanding the clinic to ED discharges.
