Innovative Approaches to Improving Community Health

ProMedica, a locally owned nonprofit health system serving northwest Ohio and southeast Michigan, set out to improve community health outcomes by addressing the social determinants of health. Facing persistent needs around hunger, housing, education, and other social and economic factors, ProMedica looked for a more innovative, population-health approach to care. NRC Health is identified as the vendor associated with this case study.

With support from NRC Health-related approaches and tools, ProMedica implemented universal screening for social determinants, connected patients to direct services and community partners, and built programs such as a food pharmacy, population health institute, neighborhood initiatives, and economic development partnerships. The results included a 4% reduction in ER use and a 53% reduction in readmissions for food-insecure patients who were connected to services, along with a 30% decrease in per-member-per-month costs for those who received resources. ProMedica also screened hundreds of thousands of patients, distributed more than 315,000 pounds of food, and expanded national collaboration through The Root Cause Coalition.
