Youth Villages is a national nonprofit specializing in children’s mental and behavioral health that created LifeSet to help young people aging out of foster care. Faced with thousands of youth turning 18 each year without stable homes and at high risk for homelessness, unemployment and educational disengagement, the organization built an intensive, young-adult driven program to bridge the transition to successful adulthood.
LifeSet provides 24/7, community-based support for 17–22 year olds—focusing on self-sufficiency skills, stable housing, education, employment and relational permanency—and has scaled through strategic partnerships. By using the myEvolv EHR to document services, share forms and analyze outcomes, Youth Villages and its partners now serve more than 700 young adults daily, reached nearly 5,000 in fiscal 2020, and have helped over 20,000 young people since 1999.
