Wyandotte County Public Health Department, an innovative local health agency in Kansas, sought to break down program silos and treat the whole person by coordinating women’s health, family planning and STD services under a single Clinical Services unit. The challenge was extending that integrated care beyond the clinic walls—sharing timely, secure patient information with hospitals, primary care providers and a nearby FQHC to avoid duplicate visits and improve transitions of care.
Wyandotte deployed Netsmart’s CareConnect™ integration engine to exchange CCDs and secure messages with external providers, enabling point-to-point referrals and pre-transfer clinical communications. Early results include faster, more convenient visits (e.g., same-day IUD care coordinated with the FQHC), improved preparedness for receiving providers, higher consumer and clinician satisfaction, and anticipated cost savings from reduced redundant tests and screenings—while formal ROI measurement is still in progress.
