West End Family Counseling Services is a nonprofit behavioral health provider in Ontario, CA offering services to children and adults across three sites and more than 25,000 hours of direct care. The agency relied on paper-based charts—often for clinicians working offsite—creating long, inefficient documentation workflows, limited access to client records, and barriers to coordinated care.
West End implemented Netsmart’s myEvolv Essentials EHR and KPI Dashboards, moving records to a cloud-based, scalable system that allows simultaneous access, easier reporting, and customizable analytics. The change eliminated much of the printing and filing burden, sped up post-visit documentation, improved care coordination and data sharing, and made it simpler to meet state and county reporting and track clinical and operational KPIs.
