How a statewide approach leverages shared technology to drive outcomes and lower cost

The Missouri Behavioral Health Council (MBHC), which represents 33 nonprofit community behavioral health agencies serving more than 250,000 clients, led a statewide rollout of the Certified Community Behavioral Health Clinic program (CCBHO). The coalition faced a major challenge: coordinating care, quality reporting and value‑based Medicaid payment across 201 service locations that used 11 different EHRs, with data siloed in disparate systems and care managers spending excessive time on manual reconciliation.

MBHC implemented Netsmart CareManager as an EHR‑agnostic population health platform and built an enterprise data warehouse to aggregate claims, clinical, hospital alerts and social‑determinants data into real‑time risk profiles. The shared system automated reporting and hospitalization follow‑up, freed care managers from up to 40% of data‑search time, enabled statewide analytics and care coordination, reduced costs through economies of scale, and helped expand CCBHO coverage and improve outcomes across the state.
