Feld Care Connects is an in‑home therapy provider based in Westlake Village, Calif., delivering physical, occupational, speech and dietary services through partnerships with hundreds of home health agencies and more than 12,000 patients since 2013. The organization’s paper‑based documentation and manual fax/email workflows couldn’t keep pace with growing patient volume or support timely information sharing between therapists, the central office and partner agencies, creating a pressing need to digitize records and standardize communications.

Feld Care Connects implemented DeVero (a Netsmart EHR) for its ease of use and flexible, customizable forms that accommodate diverse populations and workflows. Clinicians now document electronically in patients’ homes while partner agencies access notes in real time, eliminating paper handoffs, improving clinician collaboration and care coordination, enhancing patient monitoring, and enabling the organization to expand services and geographic reach with confidence.
