COMTREA is a Federally Qualified Health Center serving Jefferson County, Missouri and more than a dozen surrounding counties with primary care, dental, behavioral health and community services for over 45 years. Faced with siloed workflows and cumbersome intake and referral processes, the organization had been forcing behavioral health onto a primary care EHR and maintaining multiple disparate systems, which created inefficiencies, duplicated effort and risked gaps in patient care.
COMTREA partnered with Netsmart to implement the myAvatar EHR and CarePOV Medical Note, creating a single shared record that consolidates labs, medications, vitals, allergies, referrals and more. The integrated platform streamlined documentation, supported NCQA PCMH accreditation efforts, improved point-of-care decision making and enabled smoother workflows—helping COMTREA coordinate care effectively across its behavioral health services (nearly 700 patients) and 38,000 annual primary care visits.
