Care Plus New Jersey is a full‑service community behavioral health center that pursued Certified Community Behavioral Health Center (CCBHC) status to deliver integrated, value‑based care. The organization faced operational challenges getting clinicians and workflows to meet new CCBHC requirements—standardized assessments (PHQ‑9, AUDIT), real‑time data collection, and care coordination—while aligning behavioral health and primary care practices and preparing for value‑based reimbursement.

Care Plus NJ formed a core CCBHC team, retrained staff, redesigned workflows and deployed technology tools (myEvolv, CarePathways reporting, KPI dashboards and expanded telehealth) with embedded clinical decision support. The changes enabled real‑time monitoring and reporting, increased clients receiving care coordination, reduced waitlists and drove measurable outcomes: 32% more clients on Medication‑Assisted Treatment, a 12% rise in Addictive Services clients, consistent use of PHQ‑9/AUDIT and 97% of consumers saying they would recommend Care Plus NJ.
