Rapidly scaling COVID-19 rehabilitation care journeys for Nuffield Health

Nuffield Health, the UK’s largest healthcare charity, needed to rapidly scale a specialist 12‑week COVID-19 rehabilitation programme to support people with long COVID without overloading clinicians. They partnered with Lumeon to provide a digital care coordination platform to manage a hybrid virtual and in‑person pathway and to capture patient‑reported outcome measures (PROMs) at scale across multiple sites.

Lumeon integrated with Nuffield Health’s Salesforce system to automate referrals and pre‑assessment, deliver baseline and ongoing PROMs (D12, EQ‑5D, GAD‑7), manage cohort starts and appointments, and send patient reminders. Deployed across 40 live sites in one month, the Lumeon solution saved care‑team time, made patient progress easy to track, and supported measurable improvements such as reduced breathlessness and anxiety and better physical fitness.
