Improving postnatal discharge for better patient experience

Chelsea and Westminster NHS Foundation Trust, which runs one of the U.K.’s largest maternity services, faced poor postnatal patient experience caused by delayed discharges, inconsistent information from different staff, and time-consuming manual coordination. To tackle this, the Trust engaged Lumeon to deliver a digital end-to-end care pathway using the Lumeon Care Pathway Management solution to streamline processes and standardise postnatal discharge.

Lumeon implemented a pathway mapped to the Trust’s SOP with real-time dashboards (replacing whiteboards), electronic forms, automated tasking and timers, and auto-generated reporting to synchronise staff activity and improve data quality. The Lumeon solution reduced discharge variability and mean waiting times toward the Trust’s <4‑hour target, liberated more than 1,000 bed days per year, and delivered better visibility, communication and patient experience across the postnatal ward.
