A Luma Health Case Study
Near North Health, a Chicago community health center serving neighborhoods it calls a “dignity desert,” needed to rebuild patient connections and increase access after COVID-19 disrupted care. They partnered with Luma Health to deploy an automated patient engagement platform—texting, intake forms, broadcast messaging, self-scheduling and automated workflows—to reach patients who might not get mailed notices and simplify access to care.
Luma Health implemented automated outreach, intake forms, Medicaid redetermination reminders, and campaign messaging (including a diabetes prevention program) so patients can self-schedule and staff can reallocate opened slots. The results: a 107% increase in form completion, a 300% rise in diabetes prevention program engagement in six months, and more than 4,000 patients a month receiving broadcast messages—measurable improvements in retention and reduced care gaps for Near North Health.
