ACO Increases Access to Preventive Health Screenings

Meritage Medical Network, a network of over 730 physicians serving more than half a million patients in Marin, Sonoma, and Napa counties, faced a high volume of patients needing preventive care but lacked an efficient, direct way to reach them. To address this, Meritage partnered with Luma Health and used Luma Health’s multilingual, mobile-first messaging for population health to target outreach for screenings like mammograms, colonoscopies, and diabetic eye exams.

Luma Health implemented automated text reminders and targeted preventative care messaging to patient cohorts with identified gaps in care. As a result, Meritage saw a 12.7% increase in mammograms and colonoscopies scheduled and a 32.7% increase in diabetic eye screenings scheduled, demonstrating measurable improvements in ACO quality measures and patient access to preventive services.
