The Family Clinic is a primary care practice in Opelousas, Louisiana, that needed a more capable EHR as it prepared to expand. Facing volume-driven, fee-for-service pressures that hurt staff morale and made it difficult to close care gaps and meet quality goals, the practice sought tools to better track outcomes and improve preventive care.

Using data to close gaps, coordinate care and engage patients, they expanded hospital affiliations, saw fewer 30‑day readmissions and ER visits, improved measures like blood pressure and A1c control, and were named the first CPC+ Practice of the Year within a year—now moving toward telemedicine to extend services further.
