How CCM Saved a Cardiac Patient’s Life

Pawleys Pediatrics & Adult Medicine, a primary care practice in Pawleys Island, SC, sought a way to better manage its older patients with multiple chronic conditions after learning about the CMS Chronic Care Management (CCM) program.
