Urban Health Plan (UHP) is a large network of federally qualified community health centers serving the South Bronx and Corona, NY—about 40,000 patients and more than 220,000 annual visits across multiple clinics, school programs, and shelters. Facing the need to accelerate its transformation into a more patient-centered, efficient practice, UHP sought technology to improve access to care, reduce health disparities, strengthen clinical decision support, enhance care coordination and reporting, and boost staff productivity and revenue.
UHP implemented eClinicalWorks’ EMR/PM to link sites, streamline scheduling and lab/test workflows, and expand public‑health and performance‑improvement capabilities. The system improved access and care coordination (real‑time records, referral and appointment tracking), eliminated paper chart reviews, and supported evidence‑based protocols—contributing to a more than 60% reduction in asthma hospitalizations for children ages 4–12—while increasing productivity, visits and revenue and reducing coding/billing errors.
