Bluestone Physician Services is a mobile primary care practice that delivers on-site, palliative and chronic care to frail seniors in assisted living, memory care and group homes across Minnesota, Wisconsin, and Florida. The practice needed to coordinate care across hundreds of partner agencies, manage complex chronic-care workflows, enable mobile charting and communication in facility settings, and adapt to emerging value‑based reimbursement requirements.

Bluestone partnered with eClinicalWorks, deploying its integrated EHR/PM, Chronic Care Management module, eClinicalMobile/eClinicalTouch, analytics and focused training to integrate care coordinators and streamline mobile documentation. As a result, the organization grew to 70+ providers working with 300+ partner agencies to serve thousands of patients, improved care coordination and documentation, successfully influenced expanded CCM reimbursement, and strengthened readiness for MIPS and other value‑based programs while maintaining dignified, preventive care.
