D.C. Primary Care Association Selects eClinicalWorks Unified EMR/PM Solution

D.C. Primary Care Association (DCPCA) selected eClinicalWorks unified EMR/PM solution to automate six Community Health Centers and Federally Qualified Health Centers in Washington, D.C. as part of the Medical Homes D.C. initiative. The challenge was to improve care coordination, track patients across multiple sites, and support better chronic care management, safety, and reporting for underserved residents.

eClinicalWorks implemented an integrated electronic medical records and practice management system that followed patients across care sites, streamlined enrollment and referrals, and improved billing, reimbursement, and public health reporting. The result was a more connected community health information system that enhanced continuity of care, improved preventive and chronic care management, and supported long-term sustainability for the health centers.
