Mobile Physician Services (MPS) is a 10-year-old, multi-specialty practice that delivers 100% mobile care to largely geriatric patients with multiple chronic conditions across Central Florida. Their challenge was proving that an all‑home‑visit model could deliver high-quality, cost-effective care and earn formal recognition from the National Committee for Quality Assurance (NCQA) without disrupting day-to-day operations.

Working closely with eClinicalWorks and adopting eClinicalTouch on iPads plus patient engagement tools, MPS spent two-and-a-half years aligning workflows and data to NCQA standards and achieved Level 2 Patient-Centered Medical Home recognition. The process validated their house‑call model as a potential national example, improved referrals and workflows, increased patient engagement, helped reduce costly ambulance/ER visits and hospitalizations, and generated interest from other practices exploring similar models.
