EssenMED House Calls is New York’s largest house-calls provider, founded in 2005 to deliver primary, transitional, and specialty care to chronically ill, homebound patients across NYC and Westchester. As the practice rapidly expanded, it needed a mobile, flexible, and easy-to-use electronic health record (EHR) system to support field-based clinicians, coordinate care, and manage thousands of complex patients without returning to the office to access records and test results.

In 2008 EssenMED implemented eClinicalWorks, which provided real-time chart access in the home, streamlined referrals and lab tracking, and enabled a team-based care management division. The result: greater operational efficiency, expanded services (including interim and transitional care), reduced hospital readmissions and ER visits, improved patient and caregiver quality of life, and growth to about 4,000 patients—the largest house-calls practice in NYC.
