Using community-based disease management programs to improve self-management and deliver meaningful savings

The Leading Healthy Communities Institute, a health system in Bucks County, Pennsylvania, sought to improve population health management and control costs for its adult population with chronic diseases. They partnered with Conduent and leveraged its Community Health Solutions and Healthy Communities Institute resources to implement targeted self-management education programs.

Conduent helped the client implement Stanford’s Chronic Disease Self-Management Education (CDSME) program, closely partnering with local community organizations. The solution was highly successful, with 8 out of 10 participants gaining confidence to self-manage their wellness. This resulted in substantial savings, including $740 saved per patient and a total potential savings of $2.3 million for the hospital and $13 million for the wider community.
