United Way of South Hampton Roads highlights a 23-year life expectancy gap with Conduent

The United Way of South Hampton Roads was struggling to demonstrate and address the complex challenges impacting community health. They lacked a centralized source of referenceable data, visualization tools, and a common hub for collaboration with their partners. To solve this, they reached out to Conduent and implemented its Healthy Communities Institute (HCI) Platform.

Conduent implemented its HCI Platform to create a customized community website, ghrconnects.org, which centralized data and provided powerful mapping and visualization tools. This allowed UWSHR and its partners to identify and highlight critical needs, spurring action such as a $30 million grant for community redevelopment. The site is now a valued cross-sector hub, with 25 health departments and systems using it to set their community health agendas together.
