Helping a United Way Improve Community Health Across a Local Region

United Way of South Hampton Roads was struggling to tell the story of its community's health needs. Their staff lacked a common data source, which was scattered and difficult to link together, and they had no tools for data visualization or mapping to demonstrate disparities and drive effective collaboration with partners.

Conduent implemented its Healthy Communities Institute (HCI) Platform to create a customized community health dashboard. This centralized data, provided mapping and visualization tools, and served as a collaborative hub for partners. As a result, United Way and its partners identified critical needs, including a 23-year life expectancy gap, which helped secure a $30 million federal grant. The platform is now the foundation for a health ecosystem where 25 organizations set their community health agendas together.
