CalvertHealth, a sole provider community hospital in Maryland, faced the challenge of needing continuous access to up-to-date health data to strengthen its Community Health Needs Assessment (CHNA) and increase community engagement, especially during the COVID-19 pandemic. To address this, they partnered with Conduent and implemented its Healthy Communities Institute (HCI) platform and consulting services.

The solution from Conduent provided 24/7 data access, an online survey tool, and expert consulting, which enabled CalvertHealth to gather COVID impact insights and deeply analyze community disparities. This led to a 14-fold increase in CHNA survey respondents and revealed a critical need to address food insecurity. As a result, CalvertHealth established a drive-through farmer's market to provide healthy food access, making a direct community impact based on the data-driven insights facilitated by Conduent.
