Neath Port Talbot Council’s integrated care and Reablement services support a mainly frail and elderly population and were struggling with complex team management, paper-based scheduling, limited visibility of session times, and difficulty evidencing capacity and demand — all of which hindered timely hospital discharge, reduced avoidable admissions, and measurement of progress to independence. To tackle these challenges they engaged Civica to implement Civica Scheduling.
Civica deployed Civica Scheduling across therapy and reablement teams, replacing paper processes with digital scheduling, data capture, lone‑working features and handheld devices, and integrating with existing IT systems. Civica’s solution gives Neath Port Talbot Council real‑time visibility of capacity and demand, the ability to track session counts and time to achieve independence, reduce duplication, improve staff safety and communication, and generate datasets to evidence resource needs and inform service redesign.
