University of Pennsylvania Home Care needed to improve engagement with a complex home health population and reduce avoidable rehospitalizations, especially over weekends when field staff coverage was sparse. To address this, the agency partnered with CipherHealth to deploy the Voice Episode Engagement program across a select patient census.

CipherHealth’s Voice automatically called 186 patients multiple times during their episodes to screen for issues and triage concerns to home health staff, identifying 438 issues that required follow-up. Four months after implementation, readmissions for the targeted population fell 16.3%—from 41.7% pre-Voice to 34.9% post-Voice—demonstrating a measurable impact on readmission reduction.
