To add a consistent post-visit touchpoint, UPenn Home Care partnered with CipherHealth to deploy its Voice automated care calls.
CipherHealth’s Voice solution automatically called Caring Way patients multiple times during their episodes to identify those at risk and triage reported issues to home health staff for prompt resolution. Four months after implementation, readmissions for Caring Way patients dropped 16.3%; CipherHealth’s calls uncovered 438 identifiable issues and helped 186 patients with urgent concerns, contributing to a measurable reduction in avoidable readmissions.
