Reducing Readmissions with Triage Leveraging Automated Patient Outreach to Reduce Preventable Readmissions

Large Integrated Health System, a 1,246‑bed academic health center in Indiana, faced inconsistent unit-based post-discharge follow-up and needed better transparency and workflow to reduce avoidable readmissions. They partnered with CipherHealth and implemented its Voice patient outreach platform (Voice Follow Up: Inpatient Program) to ensure every patient discharged to home received timely, standardized follow-up.

CipherHealth’s Voice automated calls and texts triaged patients to a centralized call center managed by the health system, and over Jan–Aug 2016 reached 25,041 patients with 69% engagement. Patients who engaged were 20% less likely to be readmitted, the centralized team followed up within a median six hours for reported issues, the program helped avoid 53 readmissions, and 100% of home discharges received a follow-up.
