Middlesex Hospital, a 300-bed acute care community hospital, faced the challenge of reducing costly readmissions for its inpatient and high-risk patient populations. To better predict which patients would likely be readmitted and to address underlying social determinants of health, the hospital partnered with vendor CipherHealth to implement its Post-Discharge Communications program.

Using CipherHealth's automated outreach calls, Middlesex identified key issues that were strong predictors of readmission, such as patients feeling worse, having questions about discharge instructions, or lacking transportation. By focusing follow-up resources on patients who reported these issues, they prevented an estimated 120 readmissions. CipherHealth's solution helped achieve significantly lower readmission rates, with patients who engaged in outreach calls for CHF and COPD programs being readmitted 3.4% and 9.9% less often, respectively.
