Facing the industry shift from fee-for-service to value-based care under the ACA, Pharos needed to identify patients at risk of avoidable admissions, drive consistent patient adoption across channels, and give care teams near-real-time data to intervene before conditions worsen.
Pharos built a targeted, omni-channel patient engagement program on the Aspect CXP platform—enrolling at-risk patients, delivering daily automated surveys via SMS/voice/mobile web, and routing algorithmic alerts to care teams. The results: enrolled patients kept 85% of appointments and filled 98% of prescriptions, daily engagement exceeded 90%, all-cause admissions fell more than 40%, readmissions dropped from 18% to 6% (≈67% reduction) for engaged patients, patient satisfaction topped 90% recommend rate, and one customer earned $2M in shared savings with a 5x ROI.
