a large health system reduces duplicate patient records by 87% with Zymr

The client, a large health system, faced significant challenges due to operating multiple disconnected legacy EHR platforms across five specialty departments. This lack of interoperability resulted in fragmented patient records, data silos, and clinician frustration. To modernize its digital infrastructure and unify patient data, the organization partnered with Zymr for an EHR modernization solution.

Zymr designed and implemented an enterprise-grade integration framework, which included a centralized FHIR gateway, HL7 v2 adapters, and a Unified Patient Master Index. This solution successfully migrated 2 million patient records and 50 million clinical encounters. As a result, Zymr enabled the health system to achieve a 360-degree unified patient view and an 87% reduction in duplicate records, greatly improving data accuracy, care coordination, and clinical efficiency.
