Central & North West London NHS Foundation Trust Meets Patients Where They Are With Remote Programmes

Central & North West London NHS Foundation Trust (CNWL) serves nearly a quarter of a million people across London, Milton Keynes, Buckinghamshire and prison health services. When the COVID-19 pandemic made face-to-face group rehabilitation unsafe—particularly for high-risk COPD patients—CNWL had to find a way to continue group therapies and multidisciplinary care without exposing vulnerable people.

CNWL moved pulmonary rehabilitation and other long-term condition programmes online using Zoom, backed by tech support and device delivery for patients who needed help joining sessions. The remote model preserved continuity of care, reduced travel costs and infection risk, enabled larger and more flexible groups, improved patient experience and social contact, and has been scaled to stroke, neurology and diabetes services as part of a blended, digital-first approach.
