A ZeOmega Case Study
MDwise, Indiana’s largest not-for-profit health plan with about 330,000 members—many low-income and enrolled in programs like TANF and CHIP—faced persistent preventive care gaps among high-risk patients frequently seen in community mental health centers (CMHCs). To pilot a solution with six CMHCs and enable bi-directional sharing of clinical information to boost well-child and adult preventive visits and improve diabetic LDL control, MDwise engaged ZeOmega and its Jiva population health management platform.
ZeOmega’s Jiva provided provider and member portal access and configurable reporting so CMHC staff and MDwise case managers could share a complete view of medications, ER use, gaps in care, and behavioral/medical data to coordinate interventions. The initiative delivered measurable impact in under a year: roughly 50% improvement in pediatric/adolescent well-care visits, 50% improvement in adult preventive care, and a 75% improvement in LDL among identified diabetic members, along with high user satisfaction with Jiva.
