Community Health Plan of Washington (CHP), a network of community health centers serving over 347,000 members, faced siloed case, disease, and utilization management systems that produced inefficient manual processes, weakened data integrity, and made regulatory compliance and system upgrades costly. To support a patient-centered medical home model and enable delegated care management, CHP selected ZeOmega’s Jiva population health management solution.
