Signal Health Reduces the Stress of Critical Care Decisions by using ZeOmega Healthcare Declaration Solution

SignalHealth, a clinically integrated network operated by Yakima Valley Memorial Hospital, wanted to ensure patients’ advance directives and healthcare declarations were accessible across its provider network so wishes would be honored during critical care. To solve this, SignalHealth worked with ZeOmega and its Jiva HIE to embed advance directive storage and a standardized “5 Wishes” view into the health information exchange.

ZeOmega modified Jiva HIE to support uploads and interfaced sharing of declarations, streamlined the display for quick access, and integrated the process into SignalHealth’s Compass Care Program. As a result, gaps between isolated records were bridged, families and clinicians experienced less emotional and moral distress, and 650 patients submitted advance directives within the first nine months of the rollout.
