Signal Health Provides Doctors with Consolidated Patient Information at the Point of Care with ZeOmega’s Centralized Health Information

SignalHealth, a clinically-integrated network of 16 provider organizations operated by Yakima Valley Memorial Hospital in Washington, faced the challenge of enabling nearly 30 disparate EMR systems to exchange patient information at the point of care. To create a centralized health information exchange that would deliver consolidated patient histories, lab and radiology results, and care notes to physicians, SignalHealth selected ZeOmega’s cloud-based Jiva HIE.

ZeOmega implemented Jiva HIE to aggregate and parse data from multiple EMRs into a single, user-friendly view, enable secure messaging among clinicians, and provide a special upload view for legal advance directives. The solution reduced redundant testing, accelerated access to radiology results (from as long as a week to immediate availability in clinical encounters), and over a three-month period more than 500 patients submitted their advance directives through the system—demonstrating measurable improvements in care coordination and data-driven decision making.
