MDwise, the State of Indiana’s largest not-for-profit health plan serving more than 330,000 low-income and vulnerable members, faced the challenge of extracting maximum value from lengthy, state-required Health Needs Assessments (HNAs) and making that information accessible across the enterprise. To streamline workflow and better coordinate care, MDwise turned to Jiva™ from ZeOmega to centralize HNA collection and sharing.

Using ZeOmega’s Jiva, MDwise integrated HNAs directly into the platform so remote CSRs can complete assessments via web browser, push results to member and provider portals, and apply branching, stratification and automated triggers to flag risks and route cases to care managers. The Jiva implementation expedited interventions, increased staff efficiency, boosted patient engagement and education, enabled detailed state reporting, and helped MDwise raise its HEDIS compliance rates.
