Kern Health Systems Selects Jiva™ From ZeOmega® to Transform Care Management

Kern Health Systems, the largest managed Medi‑Cal health plan in Kern County serving over 240,000 members, faced fragmented care data spread across four siloed legacy systems (UM, CM, DM and pharmacy) that prevented a holistic member view and effective population health management. After an extensive RFP and vetting process, Kern selected ZeOmega and its Jiva™ population health management platform to integrate utilization management, care management, disease management, pharmacy, appeals and grievances, and other critical data.

ZeOmega implemented Jiva with configuration, customization and staff training, completing implementation in less than a year and targeting a 2017 go‑live. The platform consolidated data and delivered actionable intelligence and a unified member view, reducing administrative burden, streamlining operations, improving care coordination for Kern’s 240,000+ members, and enabling Kern to accommodate new lines of business; ZeOmega continues to partner with Kern as UM is rolled out first followed by CM, DM and appeals and grievances.
