Integrated Delivery Network Upgrades to JivaTM 6.1, Realizing Significant Improvements in Data Management, Health Risk Assessment, and Care Coordination

Integrated Delivery Network, a leading national integrated delivery network and longstanding ZeOmega client, faced the challenge of delivering high‑value care across disparate organizations, workflows, and health information systems. As the network grew, care teams struggled to access, exchange, and reconcile patient data across multiple EMRs, often re‑entering the same information and risking errors while trying to perform timely health risk assessments and proactive, multidisciplinary care management. To address these issues the network licensed ZeOmega’s population health platform Jiva and upgraded to Jiva 6.1.

ZeOmega implemented Jiva 6.1’s fourth‑generation interoperability engine, the Jiva Sentinel Rules Engine for automated risk stratification and alerts, and a Configuration Management Module that lets the network reconfigure the platform quickly. The upgrade enabled seamless aggregation and reconciliation of claims, lab, pharmacy, and EMR data, improved care coordination by surfacing high‑risk patients and gaps in care at the point of care, reduced duplicate data entry and related errors, and gave the organization agility to tune its workflows—delivering significant improvements in data management, health risk assessment, and care coordination.
