ZeOmega implemented and customized Jiva — adding a specialized dashboard and campaign builder — to ingest claims, lab, pharmacy, survey, eligibility and vendor data and surface the organization’s proprietary opportunity and risk scores. The solution automated identification and stratification of high‑impact members, enabled multi‑channel outreach and an integrated member portal for assessments and coaching, and streamlined case manager workflows, resulting in faster, more accurate targeting of at‑risk members, improved gap closure and condition management, and measurable productivity and engagement gains for the client.
