The University of Alabama at Birmingham (UAB) Medicine, a major academic medical center, faced significant staffing challenges in adequately following up with patients after discharge. Their limited resources meant patient volume exceeded their capacity for discharge planning, making it difficult to track recovery and empower patients to self-manage chronic conditions. The Emmi solution utilized automated, interactive voice calls to contact patients after discharge. This system asked questions to identify barriers to care, educated patients on self-management, and flagged higher-risk individuals for staff intervention. For Wolters Kluwer, the results were substantial: patient engagement increased by 21%, the average number of calls per patient nearly quadrupled, and the system saved the equivalent of 1.5 full-time employees annually by ensuring nurses only focused on calls that required their expertise. Furthermore, patients who engaged with the EmmiTransition program showed lower readmission rates across all measured groups.
