Summa Health System, a population health management organization in Northeast Ohio, wanted to improve its HEDIS performance by increasing the number of diabetic patients with eye exam records on file. Working with Wolters Kluwer ELM Solutions, the organization focused on closing gaps in comprehensive diabetes care, specifically for diabetic retinopathy eye exams.

Wolters Kluwer ELM Solutions implemented an EmmiPrevent automated voice response call campaign, “Diabetes Eye Exam,” over six weeks for 289 diabetic patients due for an exam or with outdated records. The interactive calls asked patients whether they had completed an eye exam and provided instructions for sending results to their provider. As a result, 23% of engaged patients updated their records, 43% had completed an exam, and Summa Health System improved its HEDIS retinopathy measure from the 44th percentile to the 39th percentile in just six weeks.
