Hallmark Health pharmacists increase interactions with patients using clinical decision support

Hallmark Health System, which operates multiple hospitals and an oncology center, wanted to move pharmacists beyond the pharmacy and spend less time relying on paper-based processes to identify medication interventions. To streamline daily work and improve patient care, Hallmark Health turned to Wolters Kluwer ELM Solutions’ Sentri7 clinical decision support platform.

Wolters Kluwer ELM Solutions implemented Sentri7 to organize and prioritize pharmacist interventions, improve surveillance, and support real-time clinical decision-making. After implementation, Hallmark Health increased total pharmacist interventions from 4,712 to 7,893 and generated $89,342 in hard and soft cost savings, while also improving antibiotic utilization, IV-to-PO conversions, and renal dose adjustments.
