Emmi helps improve population health management Centura Health

Centura Health, a health system with 17 hospitals and over 100 physician practices, faced the challenge of improving hypertension management among its patient population. Their goals were to increase the number of patients with a blood pressure reading on file and to increase the number of patients whose blood pressure was under control. To address this, they partnered with Wolters Kluwer ELM Solutions and used its EmmiEngage interactive patient engagement program.

Wolters Kluwer ELM Solutions implemented its Emmi Hypertension program, which was a 17-minute educational module emailed to patients. The results after six months were significant. Patients who viewed the program saw a 61% increase in having a blood pressure reading on file, jumping from 41% to 66%. Furthermore, Emmi viewers were 16% more likely to maintain controlled blood pressure and 15% more likely to achieve blood pressure control if they started with uncontrolled hypertension compared to non-viewers.
