A Wellth Case Study

Preview of the Staten Island Performing Provider System Case Study

Wellth decreases avoidable ED visits by 92% in high-risk diabetes population

Staten Island Performing Provider System (SIPPS) partnered with Wellth to address a challenge with its highest-risk diabetes patients on Medicaid. Their goal was to reduce avoidable emergency department visits and hospitalizations, a key objective of New York's DSRIP program, by improving disease management and medication adherence in this population.

Wellth implemented a program using behavioral economics to motivate patients. Members earned financial incentives for daily medication adherence and blood sugar monitoring, verified by photo submissions. This Wellth solution drove an 86% average daily adherence rate, a 1.29-point reduction in average HbA1c, and a 92% decrease in avoidable ED utilization, demonstrating significant clinical and cost-saving improvements.
